  • • Are you a compassionate and service-oriented individual with a passion for enhancing people's lives through exceptional healthcare support? Modivcare Inc. is seeking a dedicated Patient Engagement Specialist III to join our dynamic team and play a pivotal role in connecting patients with vital organizational programs.
  • • In this crucial remote position, you will be at the forefront of patient interaction, responsible for meticulously processing new referrals, verifying insurance eligibility, and clearly communicating patient co-pay or cost-share responsibilities. Your primary mission will be to not only educate patients about our innovative programs but to genuinely excite them about the benefits and support we offer, fostering a positive and proactive approach to their health journey.
  • • Your day-to-day responsibilities will involve conducting virtual outreach to patients referred by partnering providers or organizations. You will be instrumental in guiding them through the enrollment process, ensuring they understand every step, have their questions thoroughly answered, and that all necessary administrative tasks are completed efficiently and accurately.
  • • A key aspect of this role involves the precise and timely processing of patient information into our infrastructure and workflow tooling, including Electronic Health Records (EHR). This requires a keen eye for detail and a commitment to data integrity to ensure seamless program delivery.
  • • You will coordinate all facets of patient enrollment, from initial eligibility determination and scheduling appointments with our clinical team members to managing device orders. This comprehensive approach ensures a smooth and supportive experience for every patient.
  • • Demonstrating a consistently professional attitude, sound judgment, and profound empathy will be paramount in all your interactions. You will engage with patients, their caregivers, and other internal and external healthcare team members, building trust and rapport through your considerate and effective communication.
  • • As an integral part of a team of passionate healthcare experts, you will contribute to the development and implementation of innovative, low-risk, high-benefit programs designed to keep patients healthy, happy, and out of the hospital. Your insights and dedication will directly impact patient outcomes and the overall success of our initiatives.
  • • This role demands exceptional interpersonal skills, allowing you to inform, reassure, and adeptly navigate conversations, adapting your approach as needed to meet individual patient needs. You will work autonomously with minimal supervision while also thriving in a collaborative team environment, contributing to a shared mission of patient well-being.
  • • The ideal candidate will possess a strong drive for results, setting ambitious goals and taking calculated risks to achieve them. You will act with a sense of urgency, demonstrating discipline and integrity even when faced with challenges, showing unwavering determination.
  • • A deep customer-focus is essential. You will actively listen to understand patient perspectives, patiently address their frustrations, anticipate their needs, and commit to exceeding their expectations. Sharing ideas to enhance the patient experience and building rapport through empathy and reliability are core to this role.
  • • Self-awareness will enable you to thoughtfully consider potential outcomes and their impact on others before taking action, directing emotional energy into productive communication. You will be able to articulate your personal values and aspirations, leveraging your strengths while actively managing areas for development.
  • • Valuing others is fundamental. You will embrace the individuality of all individuals, treating everyone with dignity, respect, and compassion. Appreciating diverse cultures and perspectives, seeking common ground through listening and empathy, and crediting others for their contributions will foster strong relationships across the organization through transparency and trust.
  • • Learning agility and development are key. You will seek challenging assignments to broaden your skills, proactively utilize resources for personal growth, and reflect on successes and failures to drive continuous improvement. Seeking and incorporating feedback from peers and leaders will be a regular practice.
  • • Innovation is encouraged. You will seek and share ideas to improve work processes, embrace opportunities to pilot new approaches, and reflect on how new ideas impact others and the customer. Adapting to changing conditions and finding effective ways to accomplish tasks will be crucial.
  • • This is an exciting opportunity to make a tangible difference in the lives of patients, contributing to a mission-driven organization that values its employees and their well-being.
