📋 Description

• As a Patient Engagement Specialist at Modivcare Inc., you will play a vital role in improving patient outcomes by guiding individuals through enrollment in health programs, ensuring they understand processes, feel supported, and are empowered to take charge of their health—directly contributing to reduced hospitalizations and better community wellness.
• Day to day, you will process new patient referrals, verify insurance eligibility and cost share, conduct virtual outreach to patients referred by partner providers, educate and excite patients about organizational programs, complete enrollment administrative tasks, coordinate scheduling with clinical teams, manage device ordering, and maintain accurate records in EHR/workflow systems while demonstrating empathy and professionalism in all interactions.
• You will join a passionate team of healthcare experts at Modivcare, a mission-driven organization dedicated to developing innovative, low-risk, high-benefit programs that keep patients healthy, happy, and out of the hospital, operating in a fully remote environment that values service, empathy, and collaboration.
• In this role, you will develop strong patient communication and advocacy skills, gain experience in healthcare coordination and eligibility workflows, build proficiency with EHR systems, and grow your ability to thrive in a virtual team setting—positioning you for advancement in patient-facing healthcare roles or care coordination.

🎯 Requirements

• High School Diploma or GED required.
• Two (2) plus years of customer service, sales, or direct patient-facing experience.
• Experience working with the senior population in home health, clinics, or other health service settings (preferred).
• EHR/CMR experience (preferred).
• Exceptional written, verbal, and interpersonal communication skills.
• Ability to work effectively with minimum supervision and thrive in a fully virtual environment.
