- • Serve as the primary care provider for adult and pediatric family members in home and virtual settings, delivering comprehensive primary, acute, chronic, and preventive care tailored to each family’s needs.
- • Manage a longitudinal panel of families through in-home visits, video consultations, phone calls, and SMS, ensuring continuity of care and proactive health management.
- • Collaborate within an interdisciplinary clinical team including physicians, behavioral health counselors, registered nurses, psychiatrists, registered dietitians, and upskilled medical assistants to deliver holistic, whole-family care.
- • Utilize Nest Health’s electronic health record and care management tools to document patient interactions, track care tasks, coordinate with community resources, and maintain accurate clinical records.
- • Participate in daily team huddles and weekly case conferences to evaluate patient progress, refine Nest Action Plans (NAP), and coordinate physical and behavioral health interventions.
- • Diagnose, treat, and manage acute and chronic conditions including diabetes, hypertension, CHF, respiratory illnesses, depression, and infectious diseases; prescribe and reconcile medications appropriately.
- • Order, administer, obtain, and interpret diagnostic tests including venipuncture and phlebotomy, and ensure timely follow-up on results.
- • Educate patients and families on conditions, treatment plans, medication adherence, and preventive care in a culturally congruent and non-judgmental manner.
- • Provide advocacy and navigation support to help families access eligible health and social services, including housing, food assistance, transportation, and insurance enrollment.
- • Engage in outreach efforts to explain Nest Health’s services to potential families and encourage enrollment in the program.
- • Maintain current BLS/CPR certification and adhere to all clinical protocols and safety standards.
- • Participate in a rotating call schedule that includes occasional evening and weekend shifts each month.
- • Travel to patients’ homes across the New Orleans region using personal vehicle; carry and manage portable healthcare supplies during visits.
- • Perform extended desk work and computer-based documentation while adhering to EHR workflows and data integrity standards.
- • Lift and carry equipment weighing up to 25 pounds during home visits and while transporting supplies.
- • Communicate effectively with team members to report patient concerns, identify systemic gaps, and propose process improvements in a fast-paced startup environment.
- • Maintain clinical autonomy while proactively seeking guidance when needed, adapting to evolving workflows and organizational priorities.
- • Support vaccination education and encouragement efforts for families, including COVID-19 immunization, in alignment with Nest Health’s care philosophy.
🎯 Requirements
- • Completion of an accredited Physician Assistant program and active, unrestricted license to practice in Louisiana
- • Minimum of 2 years of clinical experience providing care to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions
- • Demonstrated proficiency in clinical nursing skills including venipuncture, phlebotomy, patient assessments, and care plan reinforcement for common conditions (e.g., diabetes, hypertension, depression)
- • Valid driver’s license, reliable personal vehicle, auto insurance, and willingness to comply with Nest’s Motor Vehicle Safety Policy
- • Ability to relocate to New Orleans, Louisiana, and work a full-time, five-day work week with rotating evening and weekend call duties
- • Current BLS/CPR certification
