- • The Quality Improvement Professional plays a vital role in auditing vendor agencies that provide service coordination to members in Humana’s Pathways program, ensuring compliance with contractual obligations and quality standards while driving improvements in service delivery.
- • Day-to-day responsibilities include conducting audits and evaluations of vendor agencies, analyzing performance metrics, developing action plans based on findings, providing training and guidance to vendors, administering person-centered assessments, contacting members via phone or in-person to establish goals and care plans, coordinating LTSS services, supporting members in accessing community resources, and participating in interdisciplinary care team meetings.
- • The role is part of Humana Inc., a leading U.S. healthcare company dedicated to improving health outcomes for Medicare and Medicaid members, families, individuals, and communities through its insurance and CenterWell healthcare services, with a mission to deliver the right care at the right time.
- • In this role, you will develop expertise in healthcare quality auditing, person-centered service coordination, long-term services and supports (LTSS) navigation, vendor management, and interdisciplinary collaboration — gaining valuable experience that supports career growth in healthcare quality improvement and population health management.
🎯 Requirements
- • Bachelor’s degree or equivalent
- • Must reside in Indiana
- • Prior experience in an insurance or healthcare setting
- • Experience in provider relations and education
- • Understanding of healthcare quality measures such as STARS and HEDIS
- • RN license (preferred)
