📋 Description

• • As a Quality Specialist at Shmueloff & Co. Law Offices (operating through VillageMD and affiliated entities), you will play a critical role in improving patient outcomes by ensuring medication adherence and closing gaps in care for value-based contracted patients. Your work directly supports the company’s mission to deliver high-quality, connected care that reduces total cost of care while improving health outcomes.
• • Day to day, you will perform outbound patient calls to support chronic disease management (diabetes, blood pressure, cholesterol), interpret medical histories, coordinate with clinics and pharmacies, document care activities in EMR systems, request medication refills, assist patients with medication access barriers, identify referral needs, schedule appointments and screenings, and follow up persistently to ensure medications are picked up and taken as directed.
• • You will join a physician-led, patient-centric network serving millions across traditional practices, home care, and virtual settings through Village Medical, Village Medical at Home, Summit Health, CityMD, and Starling Physicians. The team is committed to innovating value-based care using integrated applications, population insights, and staffing expertise to ensure equitable access to high-quality care.
• • In this role, you will develop strong clinical communication skills, gain hands-on experience with multiple EMR platforms, deepen your understanding of quality measure requirements and Medicare Advantage plans, and build expertise in medication adherence programs—positioning you for growth in healthcare quality, population health, or care coordination roles.
