📋 Description
- • As a Remote Nurse Health Specialist, you will provide essential triage and clinical support to seniors and chronically ill patients through Alignment Healthcare’s Virtual Care Center, helping prevent unnecessary hospitalizations and improve health outcomes by delivering timely, compassionate care 24/7.
- • Your day-to-day responsibilities include answering inbound patient calls, using clinical judgment to assess concerns, conducting outbound follow-up calls and virtual visits, reviewing daily vitals for remote monitoring patients, interpreting diagnostic tests, educating patients on disease management and medication compliance, and collaborating with physicians, nurse practitioners, and care teams to develop and adjust care plans.
