  • • Your primary responsibility will be to build and nurture robust, trusting relationships with Medicare patients. You will delve deep into understanding their unique healthcare journeys, identifying their immediate needs, and proactively addressing their concerns. This role demands a high degree of emotional intelligence and the ability to connect with individuals from diverse backgrounds, recognizing and respecting their lived experiences.
  • • A critical aspect of your role will be to master the Solace systems, tools, technology, and partner networks. While you will be expected to integrate seamlessly into our established frameworks, your unique expertise as an RN will be invaluable in enhancing our processes and client interactions. You will be instrumental in identifying and prioritizing patient needs, ensuring a streamlined and cohesive care continuum. This means meticulously tracking patient progress, coordinating with various healthcare providers, and advocating for timely and appropriate interventions.
