• • The Senior Population Health Strategy Professional is a key leader supporting the strategic direction and operational execution of population health initiatives within the Oklahoma Medicaid market, working to identify, address, and reduce health disparities, improve health outcomes, and ensure cost-effective care.
• • Day to day, the role involves leading planning, coordination, and execution of high-impact population health initiatives; applying data-driven approaches to identify health disparities and population-specific needs; collaborating with internal and external partners to promote equitable care and address social determinants of health; developing and disseminating actionable reports; supporting design and delivery of culturally relevant interventions; managing multiple projects and partnerships; supporting regulatory reporting; and occasionally traveling throughout the Oklahoma market for events and meetings.
