Social Work Case Manager

📋 Description

• • As a Social Work Case Manager at Devoted Health, you will support the care team in serving patients with complex chronic conditions, frail elders, and those at high risk of hospital admissions by coordinating services to address socioeconomic and psychosocial needs.
• • Day to day, you will establish trust and build strong relationships with patients telephonically and virtually, conduct screenings, create personalized action plans, support caregivers, facilitate end-of-life planning discussions, and collaborate with interdisciplinary teams including clinicians, community health workers, and behavioral health specialists.
• • You will be part of Devoted Medical Group’s Complex Care Department, a highly collaborative, multidisciplinary team united by a mission to build a better care delivery system for America’s most vulnerable populations, treating each member like family.
