📋 Description
- • The Social Worker role at Vynca is essential to delivering compassionate, comprehensive care to individuals with complex needs, directly supporting the company’s mission to provide more quality days at home through palliative and supportive care services.
- • Day to day, the Social Worker conducts remote psychosocial assessments via HIPAA-compliant technology, provides grief and bereavement support, screens for mental health and safety risks, participates in interdisciplinary team meetings, manages social work case loads, assists with accessing community and healthcare resources, facilitates advance care planning, completes timely documentation, and supports community outreach and education efforts.

- • In this role, the Social Worker will deepen expertise in palliative and hospice care, strengthen skills in crisis intervention and resource navigation, contribute to meaningful patient and family outcomes, and grow professionally within a mission-focused organization that values diversity, equity, and inclusion.
🎯 Requirements
- • New Jersey LCSW, LMFT, or LPCC license required
- • 2+ years of healthcare social work experience
- • Experience in hospice, palliative care, oncology, or home health settings
- • Strong understanding of cultural competency with Medicaid populations
- • Excellent oral and written communication skills with positive interpersonal abilities
- • Proficiency in MS Office, G-Suite, internet, EMR systems, and business platforms like Salesforce and Athena
