📋 Description

• Provides social work assistance to patients and families addressing social determinants of health within a home care and hospice setting.
• Conducts patient interviews to assess needs, prioritize concerns, and document all case activities accurately.
• Collaborates with interdisciplinary team members to coordinate patient care and support through community resource referrals.
• Facilitates access to community assistance by connecting patients and families with appropriate health, welfare, and social service agencies.
• Builds and maintains productive relationships with external community agencies to identify and implement creative solutions for patient needs.
• Actively participates in team discussions focused on improving standard work processes and patient care outcomes.
• Maintains a constant state of alertness and adheres to safety protocols while performing duties in diverse home and clinical environments.
• Performs essential administrative tasks including keyboard use and data entry as part of daily case documentation.
• Engages in occasional driving to visit patients in their homes, requiring a valid driver’s license and proof of motor vehicle liability insurance meeting state minimums.
• Utilizes vision and depth perception for both near (20 inches or less) and far (20 feet or more) distances during assessments and documentation.
• Relies on hearing and speech for clear oral communication with patients, families, and team members, including the ability to hear alarms or equipment malfunctions.
• Occasionally bends, stoops, climbs, twists, grips, lifts, carries, pushes, or pulls objects weighing up to 25 pounds.
• Works a 32-hour weekly schedule during the Day Shift in the 1902000116 HOSPICE FDL department.
• Operates in compliance with state-specific professional licensing requirements for Wisconsin, Illinois, Missouri, or Oklahoma.

🎯 Requirements

• Bachelor’s degree in Social Work (BSW)
• Valid driver’s license from state of issuance
• Proof of motor vehicle liability insurance meeting minimum state requirements
