Speech Language Pathologist, Home Care - Per Visit

📋 Description

• • Provides occupational therapy examination and evaluation to determine prognosis, provide intervention, and update plan of care for patients in home care settings.
• • Assists manager/supervisor with clinical and operational issues, and works with management to improve processes and utilization of rehab services.
• • Consults with medical, ancillary professionals and team members to ensure optimum quality patient care and effective operations.
• • Demonstrates comprehensive evaluation skills and interventions, including evaluating effectiveness and modifying plans of care as appropriate.
• • Maintains and completes evaluations, patient records, reports, and written documentation effectively and timely, in accordance with regulatory and department guidelines.
• • Effectively communicates with patients and/or families/significant others to provide education and literature tailored to age, physical condition, and cultural needs.
