- • You will be instrumental in coordinating care plans, working closely with community care coordinators, providers, and other healthcare team members to develop effective transition strategies.
- • A key responsibility is to educate members and their families/caregivers about the services available, as well as the specific requirements, limitations, and exclusions that may arise as a result of their transition.
- • You will actively identify new member enrollees who require specialized transition services, collaborating with care coordination and care management teams to ensure these members receive the support they need.
