• • As a Transition Specialist at Centene Corporation, you will play a pivotal role in transforming the health of our communities by ensuring the seamless and safe transition of our 28 million members through various healthcare services.
• • This role is crucial for members navigating different levels of care, including transitions into adulthood and adult services, or moving between different healthcare providers and support systems.
• • You will be instrumental in coordinating care plans, working closely with community care coordinators, providers, and other healthcare team members to develop effective transition strategies.
• • A key responsibility is to educate members and their families/caregivers about the services available, as well as the specific requirements, limitations, and exclusions that may arise as a result of their transition.
• • You will actively identify new member enrollees who require specialized transition services, collaborating with care coordination and care management teams to ensure these members receive the support they need.
• • Ensuring continuity of care is paramount; you will verify that existing authorizations are honored during the transition process and proactively work with the care management team and providers to resolve any issues that may arise.
• • You will serve as a primary point of contact and resource for members and their families, providing clear and comprehensive information to empower them during their healthcare journey.
• • The role involves tracking and maintaining key transition metrics, such as new member assessments and the volume of members transitioning in or out of care. This data will be used to identify trends, pinpoint areas for process improvement, and ensure all transition of care information is meticulously documented.
• • You will contribute to the development of educational materials and resources designed to inform members about the intricacies of transition of care services, including requirements, limitations, and exclusions.
