- • Conduct telehealth-based evaluation and management (E/M) visits and Annual Wellness Visits (AWVs) to establish medical necessity for Baba’s care navigation programs for older adults.
- • Identify high-risk medical conditions and unmet social determinants of health (SDOH) during structured intake consultations to determine patient eligibility for ongoing care support.
- • Accurately document clinical findings, diagnoses, risk factors, and SDOH Z-codes within Baba’s proprietary care platform to ensure compliance with Medicare and insurance billing requirements.
- • Validate and approve individualized care plans that align with each patient’s medical treatment plan and social needs, ensuring continuity between clinical oversight and care advocacy services.
- • Collaborate directly with care advocates and nurse care managers to finalize care plans that integrate medical, behavioral, and social support components for older adult patients.
- • Ensure all clinical documentation meets incident-to billing requirements and supports Medicare reimbursement for care navigation services.
- • Maintain adherence to state and federal telehealth regulations, HIPAA compliance, and clinical best practices during remote patient interactions.
- • Participate in ongoing quality assurance reviews to uphold clinical standards, improve intake processes, and enhance patient outcomes through data-driven care coordination.
- • Utilize telehealth platforms and EMRs efficiently to conduct secure, timely, and compassionate virtual consultations with older adult patients across multiple states.
- • Serve as a clinical anchor for care navigation programs by providing authoritative clinical judgment to guide non-clinical advocates in addressing complex patient needs.
- • Contribute to the refinement of intake protocols and clinical workflows based on real-world patient data and feedback from care teams.
- • Maintain up-to-date knowledge of Medicare Advantage, Medicaid, and value-based care models as they relate to care navigation and preventive health services for older adults.
- • Prioritize patient safety and ethical decision-making in all interactions, ensuring that care pathways are equitable, accessible, and patient-centered.
- • Respond to clinical escalations from care advocates regarding patient stability, medication changes, or emergent social needs requiring immediate clinical review.
- • Complete required continuing education and compliance training related to telehealth, geriatric care, and SDOH documentation as mandated by Baba’s clinical operations team.
- • Support scalability of Baba’s model by delivering consistent, high-quality clinical intake services across a growing national patient base.
- • Represent Baba clinically in interactions with payers, partners, and regulatory bodies as needed, ensuring alignment with evolving healthcare delivery standards.
🎯 Requirements
- • Completion of an accredited Nurse Practitioner program
- • Active Nurse Practitioner or APRN license, in good standing, licensed in multiple states
- • 2+ years of experience in primary care, telehealth, or care management
- • Familiarity with Medicare programs, including care navigation, incident-to requirements, and SDOH documentation
- • Proficiency with EMRs, care management platforms, and telehealth workflows
