📋 Description

• Conduct telehealth-based evaluation and management (E/M) visits and Annual Wellness Visits (AWVs) to establish medical necessity for Baba’s care navigation services for older adults.
• Identify high-risk medical conditions and unmet social determinants of health (SDOH) during initial intake consultations to determine patient eligibility for ongoing care support.
• Accurately document clinical findings within Baba’s proprietary platform, utilizing appropriate ICD-10 codes and SDOH Z-codes to ensure compliant billing and clinical reporting.
• Validate and approve individualized care plans developed by care advocates and nurse care managers, ensuring alignment with each patient’s medical treatment plan and clinical needs.
• Collaborate directly with care advocates, nurse care managers, and clinical operations staff to finalize care pathways that integrate medical, social, and logistical support for older adult patients.
• Ensure all telehealth interactions meet regulatory standards for Medicare-covered services, including incident-to requirements and documentation guidelines.
• Participate in structured, time-efficient virtual visits designed to screen for frailty, chronic disease burden, medication non-adherence, food insecurity, isolation, and mobility limitations.
• Maintain strict adherence to HIPAA compliance and patient confidentiality protocols during all remote patient interactions.
• Provide clinical oversight to ensure care navigation services are medically appropriate, evidence-based, and patient-centered.
• Contribute to continuous improvement of intake protocols by sharing clinical insights to refine risk stratification tools and care coordination workflows.
• Serve as a key clinical liaison between care advocates and the broader healthcare system, enabling seamless transitions into appropriate specialists, community resources, and medical equipment services.
• Support the scaling of Baba’s telehealth model by consistently delivering high-quality, compassionate, and efficient intake assessments within a remote, asynchronous workflow.
• Ensure timely completion of patient documentation and care plan approvals to maintain program compliance and minimize delays in service activation.
• Stay current with evolving Medicare policies, value-based care initiatives, and emerging SDOH screening standards applicable to geriatric populations.
• Engage in regular peer review and quality assurance processes to uphold clinical standards across the care navigation program.
• Operate independently with minimal supervision while maintaining clear communication with the clinical operations and clinical leadership teams.

🎯 Requirements

• M.D. or D.O. with an active, unrestricted U.S. medical license
• Broad multistate licensure strongly preferred (IMLC participation or 50-state coverage ideal)
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
• 2+ years of experience in primary care, telehealth, or care management
• Familiarity with Medicare programs, including care navigation, incident-to requirements, and SDOH documentation
• Proficiency with EMRs, care management platforms, and telehealth workflows
