This pivotal role is responsible for building, leading, and scaling the operational core of how our members experience our innovative cardiology programs. You will be at the forefront of designing and implementing high-performing care teams, meticulously crafting workflows that enable proactive patient engagement through outbound calls, efficient handling of inbound support requests, and timely interventions to ensure optimal patient outcomes within a value-based cardiology model.
- • This is a deeply hands-on position where your strategic vision will translate directly into tangible operational excellence. You will be empowered to design the intricate workflows that govern patient interactions, recruit, train, and develop a world-class team of care professionals, implement cutting-edge enabling technologies, and relentlessly drive performance improvements. - • To maintain visibility and control, you will be responsible for building and maintaining performance dashboards that provide real-time insights into key metrics. Furthermore, you will establish rigorous Quality Assurance (QA) programs to ensure adherence to best practices and develop sophisticated capacity planning models to anticipate and meet future demand. The establishment of Standard Operating Procedures (SOPs) and operating standards will be crucial to ensure consistency and scalability as we expand our reach nationally.

- • **Build & Lead Exceptional Care Teams:** You will be responsible for the end-to-end lifecycle of our patient-facing care teams. This includes the strategic recruitment of top talent, the development of comprehensive training programs, and the scaling of our teams to meet growing demand. You will lead nurses, care coordinators, and frontline leaders, empowering them to proactively engage patients and deliver exceptional care. Driving excellence in communication, meticulous documentation, and achieving value-based outcomes will be key performance indicators for your teams.
- • You will develop clear productivity standards, establish defined career paths to foster professional growth, and cultivate a strong, positive team culture. The goal is to build a scalable care model that achieves growth without a linear increase in headcount, demonstrating true operational innovation and efficiency.
- • **First 90 Days Focus:** In your initial three months, you will focus on standing up the core care teams with the appropriate structure, ensuring adequate capacity and leadership coverage for live operations. You will launch foundational operating standards for outreach, documentation, and quality assurance. You will activate initial live care programs, seamlessly integrated with Chamber’s platform, and implement early tooling and process improvements designed to reduce administrative burden and boost productivity. Finally, you will lead an executive-level performance readout, presenting key findings on engagement, capacity, and overall scalability readiness.
