• • The Community Guide role at Reema Health is a mission-driven position focused on building genuine, positive relationships with members to improve their health and wellbeing through personalized, community-based care. This role is vital in bridging gaps in healthcare and social services by meeting individuals where they are and connecting them to essential resources.
• • Day to day, the Community Guide will engage members through outreach, assess needs, develop care plans, coordinate referrals, conduct in-person visits (including homes, hospitals, and community settings), document interactions via the Reema App, manage a caseload of 60–100 members, and support self-sufficiency goals while maintaining professional boundaries and confidentiality.

• • In this role, the person will develop expertise in community engagement, care coordination, trauma-informed support, and resource navigation. They will strengthen skills in time management, crisis planning, documentation compliance (including HIPAA), and interdisciplinary collaboration, preparing them for advancement in public health, social work, or integrated care models.
