- • The Community Guide role at Reema Health is a mission-driven position focused on building genuine, positive relationships with members to improve their health and wellbeing by connecting them to healthcare and community resources. This role is essential in transforming how health and social care work together through a personalized, community-based approach that reduces systemic barriers to care.
- • Day-to-day responsibilities include conducting outreach to engage members from diverse backgrounds with complex social needs and chronic health conditions, assessing member needs to create personalized care plans, coordinating referrals and appointment scheduling with behavioral health and primary care providers, managing a caseload of 60–100 members through phone, text, email, and in-person visits (including homes, hospitals, and community settings), documenting all interactions via the Reema App while ensuring HIPAA compliance, and preparing members for program graduation by fostering self-sufficiency and independence.

🎯 Requirements
- • Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or a related field, or equivalent combination of education and experience
- • Three or more years of direct experience working with individuals facing complex health needs and social challenges, including knowledge of community resources
- • Demonstrated proficiency in case management, crisis planning, maintaining professional boundaries, and ensuring personal safety in community-based work
- • Valid driver’s license, reliable transportation, vehicle insurance, and satisfactory driving record required for 25–50% travel within a 25-mile service area
- • Ability to lift and carry up to 25 pounds occasionally
- • Proficiency in Microsoft Word, Outlook, Excel, and video conferencing; G-Suite preferred
