- • The Community Guide role at Reema Health is a mission-driven position focused on building genuine, positive relationships with members to improve their health and wellbeing through personalized, community-based care. This role is integral to Reema’s vision of transforming how health care and social care work together by meeting members where they are and navigating complex systems on their behalf.
- • Day to day, the Community Guide will engage members from diverse backgrounds with complex social needs and chronic health conditions, conduct outreach to enroll individuals in Reema’s care model, assess needs, create member-centered care plans, coordinate referrals to behavioral health and primary care, manage a caseload of 60–100 members, conduct in-person visits in homes or community settings, utilize multiple communication channels (phone, text, email, face-to-face), connect members to community and health resources, and maintain accurate documentation using the Reema App while ensuring HIPAA compliance.
