- • Manage a panel of families as the primary care provider to deliver longitudinal primary and preventive care through in-home and virtual modalities including video, phone, and SMS.
- • Practice alongside an interdisciplinary team of MDs, LCSWs, RNs, psychiatrists, and medical assistants to coordinate whole-family health outcomes.
- • Utilize Nest Health’s electronic health record and care management tools to document patient interactions, track tasks, collect clinical data, and communicate with team members and community resources.
- • Attend daily team huddles and weekly case conferences to advise on physical health needs, establish patient health goals, and align care plans across disciplines.
- • Propose and manage treatments for chronic and infectious diseases, including prescribing and reconciling medications, and educating patients on condition management and medication adherence.
- • Order, administer, obtain, and record diagnostic tests such as labs and point-of-care assessments.
- • Provide advocacy, navigation support, and education to families regarding eligibility and access to health and social services, including vaccinations and behavioral health resources.
- • Participate in outreach efforts to explain Nest’s care model and program benefits to potential families.
- • Maintain active clinical documentation and compliance with all regulatory and organizational standards in a hybrid care environment.
- • Travel to rural communities across Louisiana for in-person patient visits, including home-based care, while also delivering virtual care services.
- • Carry portable healthcare supplies during field visits and comply with all Nest Motor Vehicle Safety Policy requirements.
- • Work a full-time schedule of four days per week, 10 hours per day, including participation in a rotating call schedule that includes occasional evenings and weekend days each month.
- • Perform clinical nursing skills including venipuncture, phlebotomy, patient assessments, and care plan reinforcement for common adult and pediatric chronic conditions such as diabetes, hypertension, CHF, respiratory concerns, and depression.
- • Demonstrate independent clinical decision-making and problem-solving in dynamic, resource-constrained settings.
- • Maintain current BLS/CPR certification at all times.
- • Serve as a leader within the interdisciplinary care team, coordinating care delivery and communicating changes effectively across functions.
- • Educate and encourage vaccination against COVID-19 as part of routine patient care.
- • Adapt to a fast-paced startup environment with evolving processes, limited structure, and high autonomy, requiring self-directed learning and proactive identification of operational gaps.
- • Collaborate frequently with team members to give and receive feedback, ensuring seamless care coordination across clinical, social, and administrative functions.
🎯 Requirements
- • Completion of an accredited Physician Assistant or Nurse Practitioner program with an active, unrestricted license to practice in Louisiana
- • Minimum of 2 years of clinical experience providing care to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions
- • Demonstrated proficiency or willingness to train in clinical nursing skills such as venipuncture, phlebotomy, and chronic condition care plan reinforcement
- • Valid driver’s license, personal vehicle, auto insurance, and ability to travel to rural homes across Louisiana
- • Current BLS/CPR certification
- • Availability to work full-time (4 days/week, 10 hours/day) and participate in rotating on-call schedule including evenings and weekends
