- • The Care Guide role at Leap Technologies is essential to ensuring patients receive seamless, informed, and empowering support throughout their specialty medication treatment journey. As the central point of contact, you will directly impact patient outcomes by coordinating care, educating on therapies, and bridging communication between patients, providers, and nursing teams.
- • Day to day, you will serve as the primary contact for patients, responding to inquiries with empathy and clarity; educate patients and providers on home infusion processes including medication handling and administration; schedule and confirm treatment appointments with nursing agencies; verify medication receipt and patient understanding before treatment; ensure clinical documentation is shared with providers post-infusion and billing details are routed to finance; and collaborate with the Customer Success team to uphold service excellence and strong client relationships.

🎯 Requirements
- • 3–5 years of nursing or pharmacy experience, preferably with infusion therapy or specialty medications
- • Active clinical license or certification: Registered Nurse (RN), Certified Pharmacy Technician (CPhT), or Medical Assistant (MA)
- • Strong patient coordination or case management background
- • Excellent written and verbal communication skills
- • High level of organization and attention to detail; able to juggle multiple responsibilities seamlessly
- • Comfortable working remotely and independently
- • Available to work full-time Pacific Standard Time (PST) hours
- • A strong patient-first mindset, with a focus on improving care experiences and outcomes
