Full Job Description

📋 Description

• Manage a caseload of 60–70 youth in the care of DCFS
• Complete required assessments to identify short/long-term behavioral and physical health needs
• Develop and maintain individualized plans of care with the Interdisciplinary Care Team (ICT)
• Conduct contractually required assessments and document findings
• Identify member needs and gaps in care (behavioral & physical)
• Monitor progress and adjust care plans as needs change

🎯 Requirements

• Master’s degree in Behavioral Health, Social Work, or a Degree from an Accredited School of Nursing
• 2–4 years of related experience
• Illinois Youth Care plan: Graduate degree and independently licensed behavioral health clinician (LCSW, LCPC, PsyD) in Illinois or Bachelor’s Degree and IL RN licensure
