- • Proactively build and maintain relationships with hospital discharge planners, case managers, and other referral sources to increase patient starts and sales of Option Care’s home infusion services.
- • Educate patients, families, and facility staff about home and alternate site infusion care options using sales techniques to facilitate informed care decisions.
- • Collaborate with internal clinical and intake teams to assess patient eligibility for home infusion therapy and ensure seamless transition from hospital to home care.
- • Collect, review, and complete all preadmission documentation, including insurance verification and medical record data, and coordinate signoff with relevant parties.
- • Serve as the primary point of contact between hospitals, patients, families, and internal teams to coordinate care transitions and ensure accurate treatment plan implementation.
- • Facilitate the transfer of medical orders via phone, fax, and e-prescribing systems to initiate therapy and support timely patient care.
- • Partner with Account Managers and Regional Sales Directors to develop and execute local business growth plans aimed at expanding referral networks and service adoption.
- • Coordinate with marketing staff to organize educational and promotional events targeting patients, caregivers, and healthcare providers.
- • Review patient medical records to extract pertinent clinical and payor information and relay it accurately to the patient registration department.
- • Ensure compliance with legal and regulatory standards for patient confidentiality and data security, including adherence to URAC and ACHC guidelines.
- • Arrange for necessary medical supplies and equipment to be delivered and available at the patient’s home or alternate site prior to therapy initiation.
- • Provide oversight and input to care providers regarding patient transition logistics and continuity of care.
- • Maintain daily communication with Account Managers to report on referral opportunities, progress against goals, and feedback on service delivery.
- • Travel to hospitals, medical offices, and care facilities to conduct in-person education, process referrals, and support referral source relationships.
- • Utilize Microsoft Word, Excel, PowerPoint, email, and internet tools to manage documentation, communications, and reporting tasks.
- • Operate with a high degree of autonomy while adhering to company policies and clinical protocols in a part-time, remote capacity (Saturdays and Sundays).
🎯 Requirements
- • High School Diploma with 2 years of experience in home infusion or post-acute infusion services, OR 1 year of similar experience with an associate’s degree or LPN license
- • Experience establishing and maintaining relationships with healthcare providers, discharge planners, and case managers
- • Proven ability to use sales techniques to educate patients and referral sources about clinical services
- • Access to reliable transportation and a valid driver’s license with proof of insurance for travel to hospitals and care facilities
- • Basic proficiency in Microsoft Office Suite (Word, Excel, PowerPoint), email, and internet systems
- • Experience applying standards such as URAC and ACHC in healthcare service delivery
