- • Join VitalCaring as a Care Triage Nurse (Remote) and make a meaningful impact by providing compassionate, timely care coordination for patients in home health and hospice settings. This role is vital to ensuring patients receive the right care at the right time, directly supporting the company’s mission to deliver exceptional, patient-centered care across 65+ locations.
- • Day to day, you will triage patient care needs over the phone, provide medical advice, coordinate with healthcare providers, manage incoming referrals, document interactions in electronic medical records, follow up with patients, and collaborate with clinicians to prevent hospital readmissions through education and timely interventions.
