- • As a Registered Nurse Case Manager 1 (RN) at 21 Health & Human Services, Inc., you will provide compassionate, high-quality home health care to patients across Santa Cruz County, directly contributing to the agency’s mission of reducing hospital readmissions and promoting wellness in the home setting.
- • Your day-to-day responsibilities include creating, implementing, and executing individualized plans of care for patients in their homes, overseeing wound care, blood draws, tube feedings, tracheostomy care, foley catheter management, congestive heart failure (CHF) management, and medication education for patients and their families or caregivers.
- • You will join a dedicated team of over 600 clinicians at 21HHS, the largest and fastest-growing home health agency in the Bay Area, recognized as a 2024 Top Workplace with hospital readmission rates consistently under 10% — well below the industry average of over 15%.
- • In this role, you will deepen your clinical expertise in complex home-based care, strengthen patient and family education skills, and contribute to a culture of excellence that has earned national recognition, including being ranked 1st among home health agencies in the Bay Area and nationwide by the San Francisco Chronicle and other major outlets.
🎯 Requirements
- • Active Registered Nurse (RN) license in the state of California
- • Minimum of one year of clinical nursing experience, preferably in home health, med-surg, or chronic disease management
- • Ability to work independently in the field, visiting patients in their homes across Santa Cruz County
- • Strong communication and interpersonal skills for educating patients and families on care plans, medications, and disease management
- • Willingness to work a rotating schedule of Tuesday–Saturday or Sunday–Thursday
- • Current BLS/CPR certification; ACLS preferred
