- • As a Lead Care Manager at Vynca, you will serve as the primary point of contact for clients with complex needs, coordinating care across medical, social, and community services to ensure seamless, compassionate, and effective support that improves quality of life and promotes independence.
- • Your day-to-day responsibilities include assessing member needs across physical health, mental health, substance use, oral health, palliative care, trauma-informed care, housing, and social supports; developing and updating client care plans with SMART goals; advocating for clients with providers; conducting outreach and engagement activities; arranging transportation; maintaining accurate records in EMR and CRM systems; completing monthly compliance reports; and providing services via telehealth, office-based, or field-based visits as needed.

🎯 Requirements
- • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
- • Willing and able to work Monday-Friday 8:30am-5:00pm Pacific Time with flexibility for potential evenings and weekends
- • Working knowledge of government and community resources related to social determinants of health
- • Clean driving record, valid driver's license, and reliable transportation
- • Excellent oral and written communication skills
- • Bilingual (English/Spanish), highly preferred
