- ā¢ As a Certified Diabetes Education Specialist - Registered Nurse at CVS Health, you will play a vital role in improving health outcomes for members living with diabetes and related conditions by providing education, coaching, and care coordination in a fully remote setting.
- ā¢ Day to day, you will conduct scheduled member appointments and outreach, assess member needs using clinical tools and data review, develop and monitor personalized care plans, and apply clinical guidelines to support optimal health status and lifestyle behavior changes.
- ā¢ You will join a dedicated team of health professionals at CVS Health committed to simplifying healthcare through innovation, compassion, and accountability, serving diverse communities with a focus on preventive and chronic care management.
- ā¢ In this role, you will deepen your expertise in diabetes education, enhance your coaching and motivational interviewing skills, gain experience in managed care and virtual health delivery, and contribute to closing care gaps while advancing your clinical and professional growth.
šÆ Requirements
- ā¢ Must be a Registered Nurse with an active and unrestricted license in state of residence, with multi-state/compact privileges and ability to obtain licensure in non-compact states
- ā¢ Must hold a Certified Diabetes Care and Education Specialist (CDCES) certification
- ā¢ Minimum of 3+ years of clinical nursing experience post licensure
- ā¢ Experience working with patients with diabetes
- ā¢ Must possess or be willing to obtain high-speed broadband internet access for telework
- ā¢ Associateās degree minimum required as a Registered Nurse
