- • As a Certified Health Coach at Sharecare, Inc., you will play a pivotal role in transforming the lives of individuals and contributing to the overall health improvement of client populations. You will be an integral part of a dynamic, multi-disciplinary team dedicated to empowering participants to make sustainable lifestyle changes and reduce their risk of chronic diseases.
- • Your primary responsibility will involve engaging with program participants through various telephonic channels. This includes proactive outbound calls to introduce program offerings and enroll eligible individuals, as well as inbound calls for ongoing support and engagement.
- • A significant aspect of your role will be providing individualized behavior change coaching. Utilizing evidence-based coaching techniques, such as motivational interviewing, you will guide members in identifying high-risk behaviors and adopting healthier habits that enhance their quality of life.
- • You will also be responsible for "chase" calls, reaching out to enrolled members who may have missed scheduled coaching sessions, ensuring continuity of care and support.
- • Collaboration is key in this role. You will work closely with other members of the care team, including nurses and external vendors, to coordinate the delivery of services and ensure a holistic approach to participant well-being.
- • You will contribute to the operational efficiency of your assigned area, ensuring that organizational and client-specific customer requirements are met. This involves managing your schedule and caseload effectively to maximize member interaction and success.
- • Caseloads can vary, operating on either a 1:1 model or a team-based approach, depending on the specific needs of the business and clients. This flexibility ensures you can adapt to diverse program structures.
- • Regular participation in daily and monthly meetings is expected. These sessions are crucial for receiving updates, sharing insights, presenting findings, and contributing to team strategy.
- • Meticulous note-taking and tracking of member progress and the efficacy of the program are essential. Your documentation will inform ongoing coaching strategies and demonstrate program impact.
- • You will be expected to learn and master documentation software, efficiently recording all interactions with members and facilitating direct communication via chat.
- • A strong emphasis is placed on attention to detail when documenting interactions, ensuring accuracy and completeness for each participant.
- • The role requires a high degree of flexibility, as you must be open to working any shift between 10:30 am and 9:00 pm CST, Monday through Friday. Additionally, you will be required to work every fourth Saturday from 8:00 am to 4:30 pm CST.
- • You will be trained to possess a deep understanding of lifestyle-related topics, including smoking cessation, weight management, nutrition, exercise, stress reduction, and sleep hygiene, enabling you to provide comprehensive guidance.
- • The ability to inspire members to embrace change and envision positive possibilities for their health journey is a core attribute of this role.
- • You will be a positive change agent, motivating individuals to alter behaviors that contribute to better health outcomes and the reduction or elimination of identified risk factors.

- • This is a remote position, requiring a stable, hard-wired internet connection, as Wi-Fi is not permitted for security and reliability reasons.
