- • Provide high-quality, individualized patient care to residents in collaboration with the nursing team at Clifton Springs Nursing Home, a CMS 5-Star facility.
- • Assist residents with activities of daily living as outlined in their individualized care plans, including bathing, dressing, grooming, and toileting.
- • Transport residents to and from activities and appointments as needed, ensuring their safety and comfort during transit.
- • Deliver leisure activities and emotional support to residents, fostering a positive and engaging environment.
- • Maintain a clean and safe resident environment by following infection control protocols and reporting hazards promptly.
- • Respond to resident requests in a timely and compassionate manner, addressing needs and concerns with professionalism.
- • Communicate effectively with families, directing them to appropriate staff members and providing updates as permitted.
- • Accurately receive and give shift reports to team members and team leaders, ensuring continuity of care.
- • Perform evaluation and reporting of resident conditions, including changes in behavior, mobility, or health status.
- • Support resident and family emotional well-being through consistent, empathetic interaction and active listening.
- • Work a scheduled shift of 7am to 3pm, Monday through Friday, with every other weekend rotation.
- • Complete 37.5 hours per week in a full-time capacity within the Long Term Care department.
- • Adhere to all state and facility regulations regarding patient care, documentation, and safety standards.
- • Follow physical requirements for medium work, including exerting up to 50 pounds of force occasionally and 25 pounds frequently, with frequent walking, standing, or squatting.
- • Participate in ongoing training and continuing education as required by department-specific care programs.
- • Accept accommodations for physical requirements as recommended by a physician or delegate, in alignment with employer policies.
🎯 Requirements
- • Minimum age of 18
- • Currently certified as a Certified Nursing Assistant (CNA) by the New York State Education Department (NYSED)
