- ⢠This role is a Chronic Kidney Disease Advanced Practice Provider (NP or PA) position at Devoted Health, Inc., focused on delivering specialized, virtual-first nephrology care to vulnerable seniors with CKD stages 4 and 5 to delay disease progression and increase planned dialysis starts.
- ⢠The position is critical to Devoted Medicalās mission of building āmicro centers of excellenceā in virtual specialty care, directly improving health outcomes for older adults by integrating clinical expertise, technology, and compassionate, family-centered care.
- ⢠Day-to-day responsibilities include delivering direct nephrology care to CKD patients, optimizing medications, managing electrolytes and volume status, facilitating shared decision-making around goals of care, and ensuring effective planning for vascular access or peritoneal dialysis catheter placement for optimal dialysis initiation.
- ⢠The provider will collaborate closely with physicians, social workers, pharmacists, and clinical assistants, while interfacing with family members and caregivers to coordinate holistic, continuous care and deliver collaborative care plans.
- ⢠Regular participation in panel review discussions to provide medical management advice and guidance, performing comprehensive assessments including diagnosis/disease review, medication review, and evaluation for STARS/HEDIS quality interventions, as well as social, home health, and DME needs.
- ⢠Utilizing Devotedās proprietary electronic health information system for patient visits, maintaining accurate and up-to-date records in compliance with legal and ethical standards, and providing feedback to improve the EHR interface.
- ⢠Engaging in quality improvement initiatives and ongoing professional development to stay current on best practices in kidney care, while adhering to all relevant laws, regulations, and telehealth and patient privacy standards.
- ⢠The team at Devoted Medical is building innovative, value-based virtual specialty care programs designed to expand access to high-quality nephrology care for underserved senior populations across multiple states.

- ⢠In this role, the Advanced Practice Provider will deepen expertise in virtual nephrology care, influence the design of next-generation CKD care models, and develop leadership in telehealth-enabled chronic disease management while making a measurable impact on patient outcomes and healthcare equity.
šÆ Requirements
- ⢠Active APRN (NP or PA) licensure in at least one of the following states: Ohio, North Carolina, South Carolina, Texas, Alabama, or Illinois, with willingness to obtain and maintain additional state licensures as needed.
- ⢠Masterās or Doctoral degree in Nursing with specialization in nephrology care, plus minimum 5 years of outpatient clinical practice and at least 2 years of direct experience in a nephrology practice or dialysis unit.
- ⢠Proficiency in telehealth technology and electronic health records (EHR), with virtual care experience preferred; bilingual English/Spanish skills and understanding of managed care, STARS/HEDIS measures, and clinical coding are strong pluses.
