  • As a Clinical Care Coordinator at Centerstone, you will play a pivotal role in delivering life-changing behavioral health services, with a specialized focus on supporting veterans, military members, and their families. This position is instrumental in ensuring these individuals and their families receive high-quality outpatient counseling and comprehensive support, bridging the gap between their needs and the available resources within the Military Services program and partner organizations.
  • In this dynamic role, you will manage a dedicated caseload of clients, meticulously documenting their progress and care plans within the electronic health record system. A significant aspect of your day-to-day will involve proactive communication and collaboration with multidisciplinary teams, ensuring a cohesive and effective treatment approach. You will be responsible for educating the community about Centerstone's valuable programming, increasing awareness and accessibility of services. A key function includes reviewing recommendations from mental health care providers, and then expertly coordinating treatment plans and identifying appropriate community resources tailored to each patient's unique situation. This involves conducting follow-up calls to assess risk levels for individuals who have expressed suicidal ideation, implementing necessary safety management strategies, and facilitating timely referrals to specialized services. You will also contribute to the vital task of managing a resource database and overseeing provider network credentialing, ensuring the integrity and efficiency of our service delivery.
  • Furthermore, you will be empowered to authorize outpatient treatment services in strict adherence to program-specific protocols, ensuring that care aligns with contractual standards and patient needs. Facilitating seamless communication and fostering collaboration among diverse community settings, as well as across internal and external teams involved in a patient's care, is paramount. This requires adaptability and a strong commitment to being an engaged team player. A significant part of your role will involve increasing positive awareness of Centerstone Military Services within various community settings, championing the mission to empower veterans and their families to thrive. This includes actively connecting with local and national organizations that offer complementary or similar resources, which will help in shaping and refining Centerstone's programs. You will also be tasked with driving eligible patients and their families to engage with Centerstone's programs, while simultaneously identifying and proactively addressing any barriers that might hinder their participation. Finally, you will contribute to program development by assessing current programs to determine their efficacy and identify areas ripe for improvement, ensuring continuous enhancement of our services.
