  • • Perform detailed documentation reviews of medical records in home health and hospice settings to ensure clinical accuracy and compliance with regulatory standards.
  • • Conduct post-acute clinical documentation audits specifically focused on home health and hospice patient records to identify gaps, errors, or missing clinical elements.
  • • Research and provide expert analysis on complex or newly issued home health and hospice billing regulations, offering actionable recommendations to clients.
  • • Evaluate client information systems to verify completeness and accuracy of clinical documentation across electronic health records and case management platforms.
  • • Lead weekly virtual client calls to present findings, outcomes, and compliance status from ongoing engagements.
  • • Analyze clinical operational trends and develop process improvement recommendations to enhance quality of care, patient outcomes, and regulatory adherence.
  • • Review documentation for focused medical reviews and draft formal appeals for denied services, including providing expert support at the Administrative Law Judge (ALJ) level of appeals.
  • • Collaborate with cross-functional clinical consulting teams to support multi-client engagements and share best practices across post-acute care settings.
  • • Maintain up-to-date knowledge of Conditions of Participation, Conditions of Payment, and standardized assessment tools such as OASIS and hospice care protocols.
  • • Prepare and deliver clear, professional written reports and presentations to clinical leaders and organizational stakeholders.
  • • Support clients across multiple time zones, occasionally requiring extended or irregular work hours outside standard business hours.
  • • Travel up to 25% of the time to client sites or company locations as needed to conduct on-site reviews or participate in training and collaboration sessions.
  • • Utilize Microsoft Office Suite (Excel, PowerPoint, Word) to document findings, create reports, and present data-driven insights.
  • • Ensure all clinical consulting activities align with Netsmart’s mission to advance care delivery through technology and operational excellence in behavioral health, human services, and post-acute markets.

🎯 Requirements

  • • Associate’s Degree in nursing, social work, physical therapy, or another related discipline
  • • Active and unrestricted license in the related clinical field
  • • At least 5 years of progressive post-acute clinical management experience in home health and/or hospice
  • • Experience with case management in home health and/or hospice, including progressive involvement in Quality Assurance activities
  • • In-depth knowledge of Conditions of Participation and Conditions of Payment, with extensive familiarity with standardized assessment documents and patient care protocols
  • • Proficient in Microsoft Office Suite (Excel, PowerPoint, Word)
  • • Excellent written and verbal communication and presentation skills
