📋 Description

  • Serve as a centralized, remote RN providing clinical backbone support to field clinical teams by managing high-volume, time-sensitive back-office clinical workflows in the EMR.
  • Review and approve new orders in the EMR, ensuring compliance with documentation standards for verbal orders and completeness of clinical documentation to meet patient care needs.
  • Conduct clinical escalation reviews at intake to support efficient referral processing and timely patient onboarding.
  • Manage end-of-episode workflows by reviewing daily discharge reports, verifying completion of discharge summaries and signed orders, confirming NOMNC documentation for appropriate discharges, and processing discipline-only or full discharge orders as needed.
  • Oversee hospital hold processes, including inpatient transfer coordination, POC completion for hospitalized patients, discharge of clients from facilities at episode end, and management of resumption-of-care workflows and pending delay approvals in the EMR.
  • Monitor and track infection control reports, identify wound score deviations, and escalate clinical concerns to leadership in a timely manner.
  • Review and trend weekly QI events to detect patterns, flag systemic issues, and recommend process improvements to enhance clinical quality and compliance.
  • Process clinical alerts and potentially avoidable event notifications, applying clinical judgment to determine appropriate action and escalation.
  • Audit missed visits and unverified missed visits daily to support appropriate care delivery, mitigate LUPA risk, and ensure proper documentation and follow-up.
  • Monitor vital sign parameter alerts for active patients, ensuring timely documentation, notifications, and clinical follow-up, escalating critical changes to branch Clinical Managers.
  • Review coordination notes for physician updates, medication interactions, and clinically significant medication issues to prevent adverse events.
  • Utilize EMR dashboards to identify change-in-condition alerts and apply clinical judgment to determine escalation pathways and interventions.
  • Drive quality, safety, and regulatory compliance by identifying recurring clinical and documentation issues and implementing process improvements to reduce errors and enhance workflow efficiency.
  • Maintain strict adherence to home health regulatory requirements across all clinical documentation, order processing, and compliance monitoring activities.
  • Operate independently in a remote environment with high autonomy, ensuring accuracy and timeliness in all clinical decision-making tasks that directly impact patient safety and billing integrity.
  • Collaborate with branch Clinical Managers by relieving administrative burdens, enabling them to focus on direct patient care, clinician coaching, and rehospitalization prevention.

🎯 Requirements

  • Active RN license (required)
  • Strong EMR proficiency with HCHB
  • Exceptional attention to detail and commitment to documentation accuracy
  • Ability to manage high-volume, time-sensitive workflows without losing quality
  • Understanding of home health regulatory requirements
  • Ability to work independently and manage time effectively in a remote environment
