📋 Description

• • The Community Guide role at Reema Health is a mission-driven position focused on building genuine, positive relationships with members to improve their health and wellbeing through personalized, community-based care. This role is vital to Reema’s goal of transforming how healthcare and social care work together by meeting members where they are and helping them navigate complex systems.
• • Day to day, the Community Guide will engage members from diverse backgrounds with complex social needs and chronic health conditions, conduct outreach to enroll individuals in Reema’s care model, assess needs, create member-centered care plans, coordinate referrals, schedule and accompany members to appointments, manage a caseload of 60–100 members, document all interactions via the Reema App, and provide in-person support in homes, communities, or clinical settings as needed.
