- • Provide home health nursing services to clients of Rochester Regional Certified Home Health Agency (CHHA) under the direction of a Registered Nurse, in compliance with Medicare, Medicaid, and New York State Department of Health regulations.
- • Accurately and timely complete all documentation related to assigned clients’ care, ensuring adherence to federal and state regulatory standards.
- • Observe and document signs and symptoms of patients’ health status, including any changes in condition, and promptly report findings to the supervising Registered Nurse.
- • Obtain the Registered Nurse’s assessment and signature on all assigned patient records prior to completion of care.
- • Deliver routine nursing services, including patient education and information, during home visits to support patient health and independence.
- • Collaborate with the Registered Nurse to revise patient care plans when patient needs are unmet or have changed.
- • Participate in the CHHA Service Education Program and maintain updated knowledge of community health practices and local resources.
- • Contribute to identifying patients’ basic physical, emotional, and cultural needs through direct observation and interaction during home visits.
- • Maintain clinical competency through ongoing training and adherence to agency protocols and standards.
- • Participate in scheduled holiday, evening, and weekend rotation as required by the department.
- • Perform other duties as assigned to support the effective delivery of home care services.
- • Model the organization’s Brand Promise and Core Values in all patient interactions to ensure superior customer service.
- • Travel to patient homes within the service area using a valid New York State driver’s license and personal vehicle.
- • Perform medium physical work requiring exertion of 20 to 50 pounds of force occasionally, 10 to 25 pounds frequently, and up to 10 pounds constantly; frequent walking, standing, or squatting is required.
- • Work a full-time schedule of 40 hours per week, Monday through Friday, 8:30 a.m. to 5:00 p.m.
🎯 Requirements
- • Graduate from an accredited School of Practical Nursing
- • Current LPN licensure in the State of New York
- • Basic Life Support (BLS) Certification from the American Heart Association (AHA)
- • Valid New York State Driver’s License and ability to pass a driver’s license record background check
