- • As a Community Outreach Specialist at Vynca, you will play a vital role in advancing the company’s mission to provide comprehensive care for more quality days at home by building and sustaining meaningful partnerships with community organizations, healthcare providers, and advocacy groups across Stanislaus County. Your work will directly impact access to Vynca’s Enhanced Care Management programs for underserved populations, helping bridge gaps in social determinants of health and ensuring vulnerable individuals receive coordinated, compassionate support.
- • You will serve as a trusted liaison between patients, community stakeholders, and Vynca’s care teams, driving referral growth, strengthening community trust, and representing the organization with professionalism and empathy in diverse settings throughout the region.
- • Your day-to-day responsibilities will include building and maintaining relationships with community stakeholders such as resource centers, medical facilities, and advocacy groups; driving referrals into Vynca’s Enhanced Care Management programs; continuously expanding your knowledge of local community services and resources through frequent engagement with agencies serving vulnerable populations; understanding local market dynamics and agency strengths to tailor outreach effectively; developing and executing strategic marketing and outreach plans aligned with organizational goals; identifying and communicating cultural, linguistic, and socioeconomic factors affecting member engagement; tracking sales and outreach activities using CRM tools and delivering compelling presentations to community partners; supporting and participating in community events while maintaining high standards of customer service; acting as a bridge between patients and healthcare practices to facilitate seamless care coordination; and strengthening referral partnerships through consistent, proactive outreach and follow-up efforts.

- • In this role, you will develop deep expertise in community health ecosystems, social determinants of health, and healthcare navigation for vulnerable populations. You will sharpen your skills in stakeholder engagement, strategic planning, CRM utilization, and culturally competent communication—positioning yourself for growth in community health, public health, or healthcare outreach leadership. You’ll also gain firsthand experience in how integrated care models improve patient outcomes, all while contributing to a purpose-driven team that values your unique background and perspective.
🎯 Requirements
- • Minimum of 2 years of experience in healthcare sales or at a community-based organization
- • Bilingual proficiency in English and Spanish (required)
- • Valid driver’s license, reliable insured automobile, and ability to travel throughout Stanislaus County (must reside within 20 miles of assigned territory)
- • Experience working with underserved communities and in healthcare settings
- • Proficiency with Microsoft and Google applications, telephone systems, and electronic health record (EHR) platforms
- • Experience using CRM systems; Salesforce experience preferred
- • Strong organizational skills, ability to work independently, and excellent communication, presentation, and motivational abilities including conflict resolution techniques
- • Access to a reliable internet connection for remote work and documentation tasks
