  • • Build and maintain relationships with community stakeholders including resource centers, medical facilities, and advocacy groups to expand Vynca’s outreach and engagement.
  • • Drive referrals into Vynca’s Enhanced Care Management programs through strategic community outreach and partnership development.
  • • Continuously expand knowledge of local community services and resources, maintaining frequent contact with agencies serving vulnerable and underserved populations.
  • • Understand and analyze local market dynamics and the strengths of community-based organizations to inform outreach strategies.
  • • Develop and execute strategic marketing plans aimed at increasing awareness and enrollment in Vynca’s care programs.
  • • Identify and communicate cultural issues affecting members to ensure culturally competent engagement and service delivery.
  • • Track sales activities and conduct presentations to community partners and stakeholders to promote Vynca’s care offerings.
  • • Support and participate in community events to strengthen Vynca’s presence and build trust with local populations.
  • • Serve as a liaison between patients, care teams, and medical practices to facilitate seamless care coordination.
  • • Strengthen referral partnerships through consistent outreach, follow-up, and relationship management.
  • • Utilize computer systems, telephones, and electronic health record (EHR) platforms to document patient interactions and schedule appointments.
  • • Operate within a field-based model requiring frequent travel throughout San Francisco County to engage with community partners and attend events.
  • • Maintain high standards of customer service and professionalism in all community-facing interactions.
  • • Ensure accurate and timely documentation of outreach activities and partnership metrics using CRM systems.
  • • Work independently with minimal supervision while aligning efforts with the goals of Vynca’s Enhanced Care Management team.
  • • Adhere to vaccination requirements, including annual influenza vaccination, for all patient- and client-facing roles.
  • • Comply with all federal and state employment verification requirements, including E-Verify upon hire.
  • • Respect and uphold Vynca’s core values of Excellence, Compassion, Curiosity, and Integrity in all community interactions.

🎯 Requirements

  • • 2 years of experience in healthcare sales or at a community-based organization required
  • • Experience working with underserved communities and in healthcare settings
  • • Valid driver's license and reliable, insured, and registered automobile
  • • Experience using CRM systems, with Salesforce preferred
  • • Proficient with Microsoft and Google applications
  • • Bilingual (English/Spanish), highly preferred
