  • • As a Complex Care Nurse Practitioner or Physician Assistant at Ennoble Care Inc., you will provide high-quality palliative care to patients in home-based settings across North Philadelphia, playing a vital role in delivering compassionate, continuous care to individuals with chronic conditions and limited mobility.
  • • Your day-to-day responsibilities include conducting patient assessments through physical exams and medical history reviews, developing personalized care plans, prescribing medications and diagnostic tests, leading sensitive conversations about goals of care and advance care planning, and collaborating with interdisciplinary care teams to ensure holistic patient support.
  • • Ennoble Care is a mobile primary, palliative, and hospice care provider serving patients across multiple states, committed to delivering trusted, continuous care in patients’ homes through programs like remote monitoring, behavioral health management, and chronic care management, guided by the motto ā€œTo Care is an Honor.ā€

šŸŽÆ Requirements

  • • Board Certified Nurse Practitioner or Physician Assistant with valid certification to practice in Pennsylvania
  • • Strong clinical skills with the ability to accurately assess, diagnose, and treat patients in home-based palliative care settings
  • • 2-3 years of experience in palliative care, chronic care management, or primary care with geriatric patients; hospice experience is highly preferred
