📋 Description

• Own end-to-end read/write integrations between Chamber Cardio’s value-based cardiology platform and practice-level EHRs using FHIR, HL7v2, and C-CDA standards, while implementing pragmatic workarounds when standards fail.
• Design and deploy clinical notifications—including GDMT prompts, care gap alerts, ADT-triggered discharge signals, and transitions-of-care events—directly into provider workflows within existing EHR systems, avoiding separate portals.
• Leverage AI tooling to extract, normalize, and route unstructured clinical data from EHRs that lack direct integration capabilities, automating manual data ingestion processes.
• Support partner and payer data integrations by ingesting eligibility files, claims data, and attribution information, and producing return files aligned with Chamber’s analytics stack (dbt, attribution logic).
• Collaborate cross-functionally with clinical, network, and practice-facing teams to translate operational needs into reliable, scalable data flows that improve care coordination.
• Build durable, maintainable integrations with robust monitoring, error-handling, and documentation to ensure system reliability as the network scales.
• Achieve fluency in Chamber’s integration sourcing, evaluation, and deployment processes within 30 days, and onboard the new practice cohort for integration readiness.
• Define and prioritize the integration strategy within 60 days, with a primary focus on securing a master integration agreement with eClinicalWorks to cover approximately 40% of Chamber’s practices.
• Establish live read access for transitions of care across 70% of patient coverage within 90 days, with foundational integrations completed for larger practices to serve as the platform’s core.
• Operate independently across the full spectrum of integration approaches—from clean API-based connections to manual, scrappy workarounds—without requiring engineering support.
• Apply SQL, Python, or AI tools like Claude to automate repetitive tasks and increase personal and team effectiveness by threefold, demonstrated through a basic coding assessment.
• Maintain alignment with Chamber’s values: Low Ego, Empathy, Courage, Ownership, and Grit, ensuring all integrations are built with compassion, integrity, and urgency.

🎯 Requirements

• 5-7+ years of experience in healthcare technology, EHR implementation, or interoperability
• Deep, hands-on experience with healthcare interoperability standards: FHIR, HL7v2, and C-CDA
• A track record integrating with multiple EHR systems at the practice or health-system level
• Comfort operating across the spectrum from clean API integrations to scrappy, get-it-working approaches
• Fluency with clinical and value-based care concepts: eligibility, claims, attribution, transitions of care, and GDMT
• AI-native and genuinely curious about automation; ability to automate tasks using SQL, Python, or tools like Claude and pass a basic coding assessment
