📋 Description
- • As an Exercise Physiologist at Ansible Health, you will deliver remote clinical care to patients with advanced chronic lung and cardiac conditions, directly contributing to improved health outcomes for a medically complex population that faces frequent hospitalizations and psychosocial challenges. Your work will be central to Ansible’s mission of transforming healthcare delivery by integrating clinical excellence with technology to provide personalized, scalable care for underserved patients.
- • You will conduct individual and group patient education sessions on disease management, medication adherence, smoking cessation, preventive care, and lifestyle modifications, while also assessing, triaging, and escalating care for patients experiencing worsening symptoms or acute exacerbations, ensuring timely interventions and coordination with primary care providers and specialists.
- • You will serve as the continuous point of contact for patients and families across the full care continuum—from post-acute transitions and medical stabilization to rehabilitation, advanced care planning, and end-of-life support—while advocating for patients facing social determinants of health by connecting them to community resources for medications, transportation, nutrition, and safe housing.
- • You will collaborate multiple times daily with supervising nurse practitioners and physicians, participate in care coordination with external agencies, and promote Ansible’s program to referring providers and local practices, blending clinical expertise with education, case management, advocacy, and outreach in a uniquely cross-functional role.
- • You will engage in pulmonary telerehabilitation, nutritional guidance, and care optimization efforts—such as coordinating transportation for patients in rural areas with limited connectivity—and contribute to meaningful clinical improvements, including increased functional status, reduced hospitalizations, and successful transitions to hospice or transplant eligibility, as demonstrated in Ansible’s recent patient success stories.

🎯 Requirements
- • Bachelor’s degree in Exercise Physiology, Exercise Science, Kinesiology, Nursing, or a related field
- • 5+ years of experience as an Exercise Physiologist or RN, with background in acute care, rehabilitation, long-term care, home care, and outpatient settings
- • Demonstrated experience with cardiac and pulmonary rehabilitation programs
- • Certified in Basic Life Support (BLS)
- • ACSM Certified Clinical Exercise Physiologist (ACSM-CEP) or Certified Cardiac Rehabilitation Professional (CCRP) or RN with relevant cardiac or pulmonary rehabilitation experience
- • Proficiency with telemedicine and/or remote patient care platforms
- • Reliable high-speed internet connection (minimum 25 Mbps)
