  • • Provide direct patient care to adults and children in home settings under the supervision of licensed clinicians, including taking vital signs, collecting lab specimens, and assisting with clinical exams.
  • • Administer oral, topical, and injectable medications and vaccinations in accordance with provider directives and infection control standards.
  • • Conduct patient interviews to document medical histories and administer standardized screening questionnaires accurately.
  • • Perform procedures using aseptic techniques and adhere strictly to PPE protocols, hand hygiene, and isolation procedures for infectious disease management.
  • • Maintain accurate, timely EMR documentation of all patient interactions, services rendered, and clinical observations per Nest Health standards.
  • • Coordinate and assist with patient registration, administrative workflows, and prior authorization approvals for clinical services.
  • • Manage clerical tasks including scanning, sending, and retrieving medical records, diagnostic reports, hospital notes, referral information, and lab requisitions.
  • • Participate in vaccine and medication inventory management and ensure compliance with regulatory and safety requirements for equipment and lab tests.
  • • Act as a patient advocate by providing navigation support, education, and resources to families on health and social services, including food insecurity, housing, transportation, and domestic violence support.
  • • Follow up with families to confirm access to recommended external social care providers and ensure continuity of support.
  • • Collaborate with interdisciplinary care teams to proactively contact families and connect them to needed providers and resources aligned with their health goals.
  • • Participate in outreach efforts to engage potential Nest families, explain program benefits, and encourage enrollment in Nest Health services.
  • • Demonstrate strong interpersonal skills when interacting with patients, clinicians, nurses, and external service providers to build trust and foster relationships.
  • • Uphold professional standards in appearance, demeanor, and communication across all patient and team interactions.
  • • Comply with all Nest Motor Vehicle Safety Policy requirements and maintain reliable transportation for home visits across the Mesa, Arizona service area.
  • • Lift or pull up to 40 pounds as required for equipment, supplies, or patient assistance during home visits.
  • • Educate patients and families on the importance and availability of COVID-19 vaccinations as part of preventive care initiatives.
  • • Work prolonged periods at a desk using computers for documentation, communication, and administrative tasks while maintaining mobile service delivery.

🎯 Requirements

  • • Successful completion of an accredited Medical Assistant program
  • • High School Diploma or equivalent
  • • 1-3 years of relevant Medical Assistant experience
  • • Active BLS certification
  • • Vital Signs and phlebotomy experience (1 year)
  • • EMR Systems experience (1 year)
