📋 Description

  • • As a Field Care Manager, Behavioral Health for Humana Inc., you will serve as the primary point of contact for Medicaid members in Illinois, delivering integrated, person-centered behavioral health care management to improve outcomes, reduce care gaps, and support vulnerable populations including those with serious mental illness, substance use disorders, and justice-involved individuals.
  • • Your day-to-day responsibilities will include conducting face-to-face and telephonic member engagements, coordinating care across medical and behavioral health providers, leading interdisciplinary care team meetings, addressing health-related social needs, improving health literacy, managing crisis interventions, and ensuring compliance with IDHS, CMS, and NCQA regulations while traveling up to 75% of the time across the south or southwest Chicago metro area.
  • • You will join Humana’s IL Medicaid Care Management team, a mission-driven unit committed to putting health first for underserved populations, where collaboration with internal departments, community organizations, and providers ensures seamless, culturally competent care delivery aligned with Humana’s vision of whole-person well-being.
  • • In this role, you will deepen your expertise in behavioral health case management, gain leadership experience in coordinating interdisciplinary teams, enhance your crisis intervention and motivational interviewing skills, and contribute directly to reducing health disparities in Illinois’ FIDE (Frailty, Intellectual/Developmental Disability, and Elderly) population through evidence-based, trauma-informed practices.
  • • You will develop proficiency in navigating complex Medicaid and Medicare systems, leveraging EHR platforms, documenting care plans with precision, and advocating for members’ needs across social service networks — building a career foundation in public health care management with opportunities for certification advancement and internal mobility within one of the nation’s leading health and well-being companies.

🎯 Requirements

  • • Active Illinois licensed LCSW, LMFT, or LCPC (no supervisee or provisional licenses)
  • • 2+ years of post-degree clinical experience in a behavioral health setting
  • • Case management experience working with complex SMI, SUD, or SED populations
  • • Ability to travel to region-based facilities and homes for face-to-face assessments (valid driver’s license, car insurance, and reliable transportation required)
  • • Intermediate to advanced computer skills with proficiency in Microsoft Word, Outlook, and Excel
  • • Residency in the south or southwest Chicago, IL area (required for regional base)
