📋 Description

• • As a Field Care Manager, Behavioral Health 2 at Humana Inc., you will assess and evaluate members' behavioral health needs to guide them toward appropriate resources, ensuring optimal wellness and facilitating access to care.
• • Day to day, you will conduct member assessments, develop care plans, resolve barriers to care, coordinate with healthcare providers, and travel up to 75% of the time to meet members in their homes or communities across South Carolina.
• • You will join Humana’s Care Management Behavioral Health team, reporting to a Manager, Care Management Behavioral Health, within a mission-driven organization focused on improving health outcomes for Medicare, Medicaid, and underserved populations.
• • In this role, you will deepen your expertise in behavioral health case management, strengthen clinical documentation and care coordination skills, and contribute to member wellness while gaining experience in a large, national healthcare leader committed to equity and innovation.
