• • The Field Care Manager RN assesses and evaluates member needs to achieve optimal wellness by guiding members and families toward appropriate resources for care and wellbeing.
• • Day-to-day responsibilities include identifying and resolving care barriers, monitoring patient progress through assessments, creating care plans, communicating with internal and external stakeholders, and visiting Medicaid members in homes, supportive living facilities, and long-term care settings with 75-90% local travel.
