- • Provide patient-centered, evidence-based nursing care in patients' homes across Manitowoc County and surrounding areas, adhering to the scope of practice defined by the Wisconsin Board of Nursing and ThedaCare policy.
- • Utilize the nursing process to assess patients' physical conditions and nursing needs, developing individualized care plans in collaboration with patients and the interprofessional care team.
- • Deliver specialized, age-appropriate, developmentally sensitive, and culturally specific nursing care consistent with clinical specialty requirements.
- • Collaborate with physicians and other healthcare professionals to problem-solve, improve patient outcomes, and enhance the overall patient experience.
- • Plan and coordinate care needs with the interprofessional team to ensure high-quality clinical outcomes and continuity of care across the healthcare continuum.
- • Identify opportunities to streamline care processes, reduce costs, and improve satisfaction for patients, families, and team members.
- • Demonstrate clinical expertise in managing complex patient conditions and proactively anticipate and manage risks to prevent crises.
- • Perform skillfully during life-threatening emergencies, effectively matching available resources to urgent clinical demands.
- • Maintain accountability for patient safety and quality outcomes while promoting a culture of innovation, mentoring, and professional growth among clinicians.
- • Participate in on-call responsibilities including 2 holidays (1 major, 1 minor) with variable hours, every 5-6th weekend on-call, and occasional evening shifts.
- • Operate a fleet vehicle when eligible to travel between patient homes within the assigned service area.
- • Adhere to scheduled work hours of Monday–Friday, 8:00 AM–4:30 PM, with one day off every other week (day TBD).
- • Perform physical tasks including standing, walking, bending, pushing, pulling, and lifting up to 50 pounds without assistance.
- • Transport, transfer, and position patients and equipment using proper body mechanics to minimize injury risk.
- • Work in environments with exposure to bloodborne pathogens, sharp instruments, communicable diseases, hazardous materials, pharmacological agents, and occasional aggressive patients.
- • Maintain compliance with all required certifications and continuing education as mandated by department and ThedaCare policy.
🎯 Requirements
- • Current Wisconsin RN Licensure
- • American Heart Association Healthcare Provider Basic Life Support (BLS)
- • BSN completion within five (5) years of hire for non-BSN nurses with less than one year of licensed RN experience
- • American Heart Association Advanced Cardiac Life Support (ACLS) within 3–6 months of hire, depending on department
- • Neonatal Resuscitation Program Certification (NRP) within 3 months of hire if assigned to OB-related roles
- • Ability to lift up to 50 pounds and perform physical tasks including standing, walking, and patient transfer
