- • Own and manage a caseload of patients delivering personalized 1:1 and occasional live group coaching aligned with clinical plans to drive sustainable behavioural change and meet patient satisfaction and retention KPIs.
- • Proactively identify engagement opportunities based on patient clinical needs, milestone moments, and coaching judgment to enhance outcomes.
- • Design and adapt coaching interventions in collaboration with clinical teams to improve patient engagement and provide feedback for service improvements.
- • Test new coaching interventions focused on retention and satisfaction, then share learnings with the Health Coaching team to inform best practices.
- • Respond to patient queries and direct concerns to appropriate internal teams within agreed SLAs and service metrics.
- • Act on patient feedback to elevate service quality and improve overall patient experience.
- • Maintain accurate, timely documentation to ensure continuity of care and support quality assurance processes.
- • Buddy and mentor new health coaches on safe, high-quality coaching practices and clinical protocols.
- • Monitor personal performance data and patient outcomes to proactively adjust coaching approaches and identify patients at risk of disengaging.
- • Participate in peer auditing and feedback sessions to uphold clinical coaching standards.
- • Monitor patient safety risks and escalate clinical concerns promptly following established escalation protocols in collaboration with the clinical team.
- • Identify and recommend clinically appropriate Juniper services and products that align with patient goals and treatment progression.
- • Collaborate cross-functionally with dietitians, shop, community, and PES teams to support the broader patient care journey.
- • Surface patient insights to the team and business to inform service improvements and operational decisions.
- • Deliver high-quality coaching from day one in a fast-paced telehealth environment without requiring extended onboarding.
- • Operate effectively in a dynamic digital health environment with evolving priorities and ambiguous workflows.
- • Apply deep expertise in motivational interviewing and behaviour change frameworks to support long-term patient adherence and outcomes.
- • Maintain strict adherence to clinical safety and governance standards while managing patient caseloads across multiple international markets.
- • Contribute to a global healthcare mission by supporting over 350,000 patients across Australia, the UK, Germany, Japan, and Canada.
🎯 Requirements
- • Accredited Practicing Dietitian (essential)
- • Proven experience managing a patient caseload in a telehealth or digital health environment
- • Demonstrated expertise in motivational interviewing and behaviour change frameworks
- • Strong understanding of medicated weight management and clinical safety protocols
- • Ability to deliver high-quality coaching from day one with minimal ramp-up
- • Comfort operating in fast-paced, ambiguous digital health environments
