- • Manage a caseload of patients and deliver personalized 1:1 and occasional live group health coaching aligned with clinical plans to drive sustainable behavioral change and meet patient satisfaction and retention KPIs.
- • Proactively identify engagement opportunities based on patient clinical needs, milestone moments, and coaching judgment to enhance outcomes.
- • Design and adapt coaching interventions in collaboration with clinical teams to improve patient engagement and provide feedback for service improvement.
- • Test new coaching interventions focused on retention and satisfaction, then share learnings with the Health Coaching team to inform best practices.
- • Respond to patient queries and escalate concerns to appropriate internal teams within agreed SLAs and service metrics.
- • Act on patient feedback to improve service quality and drive higher satisfaction scores.
- • Maintain accurate, timely documentation to ensure continuity of care and support quality assurance processes.
- • Mentor and buddy new health coaches to ensure safe, high-quality coaching practices are adopted from day one.
- • Monitor personal performance data and patient outcomes to proactively adjust coaching approaches and identify patients at risk of disengagement.
- • Participate in peer auditing and feedback sessions to uphold clinical coaching standards.
- • Monitor patient safety risks and escalate clinical concerns promptly following established protocols in collaboration with the clinical team.
- • Identify and recommend clinically appropriate Juniper services and products that align with patient goals and treatment progression.
- • Collaborate cross-functionally with teams such as shop, community, and PES to support the broader patient care journey with coaching and dietitian input.
- • Surface actionable patient insights to the team and business to inform service improvements and operational enhancements.
- • Operate in a fast-paced digital health environment with evolving priorities, maintaining high standards of care under ambiguity.
- • Deliver high-quality coaching from day one with minimal ramp-up, leveraging prior experience managing patient caseloads in telehealth settings.
- • Apply deep expertise in motivational interviewing and evidence-based behavior change frameworks to support long-term patient adherence.
- • Maintain strict adherence to clinical safety and governance protocols, demonstrating fluency in risk escalation and quality control.
- • Hold personal accountability for patient retention, satisfaction, and clinical outcome metrics as core performance indicators.
- • Work remotely across South Australia with full integration into a global, multi-market digital health team supporting patients in Australia, the UK, Germany, Japan, and Canada.
🎯 Requirements
- • Accredited Practicing Dietitian (essential)
- • Proven experience managing a patient caseload in a telehealth or digital health environment
- • Demonstrated expertise in motivational interviewing and behavior change frameworks
- • Strong understanding of medicated weight management and clinical safety protocols
- • Ability to deliver high-quality coaching from day one with minimal onboarding
- • Comfort operating in fast-paced, ambiguous digital health environments with evolving priorities
