- • As a Health Coach at Sharecare, Inc., you will play a pivotal role in a dynamic, multi-disciplinary team dedicated to enhancing the individual health and wellness of program participants, ultimately improving the overall health status of client populations.
- • You will be a key contributor to client-specific Health and Care Support Teams, actively engaging and supporting members who exhibit high-risk behaviors that could lead to chronic diseases or other health conditions.
- • Your primary objective will be to act as a positive change agent, empowering members to modify detrimental behaviors and adopt healthier lifestyles, thereby reducing or eliminating identified risk factors and fostering a higher quality of life.
- • This role involves a significant amount of telephonic communication, including engagement calls (both inbound and outbound) to inform eligible participants about program offerings and encourage enrollment.
- • You will conduct individualized behavior change coaching sessions (inbound and outbound) designed to address high-risk behaviors and promote the adoption of positive, health-conducive habits.
- • Proactive outreach will be a component of your role, with 'chase' calls to connect with enrolled members who may not have scheduled sessions, ensuring continuity of care and support.
- • Collaboration is essential; you will coordinate the delivery of services and care with a diverse team of professionals, including nurses, pharmacists, and external vendors, ensuring a holistic approach to member well-being.
- • You will contribute to the operational efficiency of your assigned area, ensuring that organizational customer requirements are met, potentially across multiple clients.
- • Maintaining a structured schedule and managing your caseload effectively are crucial for ensuring successful and consistent member interactions.
- • Caseloads may be structured as a 1:1 model or a team-based approach, depending on the specific business and client needs.
- • Active participation in daily and monthly meetings is expected, providing opportunities to share updates, present findings, and offer insights derived from your coaching interactions.
- • Meticulous note-taking and tracking of member progress, along with the efficacy of the program interventions, will be a core responsibility, contributing to data-driven improvements.
- • You will be expected to develop a deep understanding and quickly learn information related to various lifestyle topics, such as smoking cessation, weight management, nutrition, exercise, stress reduction, and sleep hygiene.
- • The role requires the ability to inspire members with enthusiasm, encouraging them to embrace behavioral change and recognize their potential for a healthier future.
- • You will learn and apply effective coaching techniques, including the principles of motivational interviewing, to facilitate meaningful and lasting behavior change.
- • Flexibility is paramount, as you must be open to working any shift between 10:30 am and 9:00 pm CST, Monday through Friday, and work every fourth Saturday from 8:00 am to 4:30 pm CST.
- • This adaptability extends to potential shift changes based on business needs, ensuring coverage and support.
- • A structured, 6-week training period from 8:00 am to 4:30 pm CST is mandatory, with no absences permitted during this time, emphasizing the importance of foundational knowledge and skill development.
- • You will be responsible for coordinating telephonic communication with program participants while simultaneously navigating a software platform and entering data, often for extended periods.
- • The ability to work independently, prioritize tasks effectively, and manage a moderate workload with minimal supervision is essential for success in this remote role.
- • You must possess the ability to multitask efficiently, managing interactions with members while also handling operational tasks with managers and support staff.
- • A stable, hard-wired internet connection is a strict requirement, as Wi-Fi usage is not permitted to ensure reliable connectivity.
