- • Complete accurate and timely analysis and reanalysis of medical records to ensure compliance and completeness.
- • Process accounts from Meditech System Waiting for Documentation and Missing Document work queues upon receipt of missing documentation.
- • Follow up with ancillary and nursing departments to obtain missing clinical documentation as specified in workflow guidelines.
- • Coordinate with providers to resolve missing documentation or dictation issues that prevent accurate medical coding.
- • Maintain and update the unbilled spreadsheet with current status of missing documentation and submit updates to management.
- • Review electronic medical record (EMR) documentation to identify deficiencies, create charting deficiency entries in the deficiency management system, and assign deficiencies to the appropriate providers.
- • Actively manage multiple electronic and manual work queues to ensure timely completion of medical record analysis and chart closure.
- • Adhere strictly to company policies, standards, and system workflow protocols when adding, reassigning, or processing accounts in work queues.
- • Identify systemic issues or recurring inefficiencies in documentation processes and escalate them to management for resolution.
- • Report any deviations from facility or corporate policies, standards, or regulatory requirements in a timely manner.
- • Document all workflow procedures, including modifications, enhancements, or changes implemented during daily operations.
- • Perform additional duties as assigned to support the efficiency and integrity of the health information management function.
- • Pursue and obtain CRCR certification within nine months of hire, with full cost covered by the company.
- • Demonstrate inquisitiveness and openness to adopting innovative technologies, including AI tools, to improve documentation processes and enhance patient and client experiences.
- • Apply knowledge of CMS and Joint Commission regulations to ensure regulatory compliance in record handling and documentation practices.
- • Utilize EMR systems effectively to manage and track medical record deficiencies and completion status.
- • Maintain familiarity with healthcare revenue cycle operations, particularly within acute care facility HIM environments.
- • Collaborate cross-functionally with clinical, coding, and administrative teams to ensure timely and accurate medical record completion.
- • Contribute to a culture of excellence by consistently delivering high-quality work in alignment with Ensemble’s “Best in KLAS” principles.
🎯 Requirements
- • Must obtain CRCR certification within 9 months of hire (company-paid)
- • Experience or familiarity with electronic medical records (EMR) systems
- • Understanding of healthcare revenue cycle processes, preferably in acute care settings
- • Knowledge of CMS and Joint Commission regulations preferred
- • Strong attention to detail and ability to manage multiple work queues simultaneously
- • Willingness to adopt and implement innovative technologies, including AI, to improve processes
