- • Join Abby Care’s mission to make family care possible by transforming how families navigate the complex world of caregiving. As a Healthcare Documentation Specialist, you will be the linchpin between families in crisis and the life-changing support they need, ensuring every critical detail is captured, organized, and communicated with precision.
- • Own the end-to-end documentation lifecycle for Medicaid waiver and long-term-care applications, medical histories, physician orders, and care plans. You will translate dense clinical jargon into clear, compliant narratives that regulators, insurers, and clinicians can act on immediately.
- • Work directly with Abby Care’s multidisciplinary team of nurses, social workers, and care coordinators to gather, verify, and synthesize information from electronic health records, insurance portals, and telehealth visits. Your meticulous eye will prevent costly delays and denials that can derail a family’s access to essential services.
- • Build and maintain a centralized, HIPAA-compliant knowledge base that evolves with ever-changing state and federal regulations. Your documentation will become the single source of truth that empowers our care advocates to secure faster approvals and better outcomes for thousands of families nationwide.
- • Serve as the quality gatekeeper for all outgoing documentation, running rigorous audits for completeness, accuracy, and regulatory alignment. You will flag inconsistencies, request missing data, and shepherd corrections through to resolution—often under tight deadlines when a loved one’s hospital discharge depends on it.
- • Design and refine templates, style guides, and SOPs that scale our documentation standards as Abby Care expands into new states and payer programs. Your work will directly influence product roadmaps, training curricula, and AI-driven automation initiatives.
- • Translate complex medical histories into empathetic, family-friendly summaries that help caregivers understand diagnoses, prognoses, and next steps. Your words will turn fear and confusion into confidence and action.
- • Collaborate with our engineering and data science teams to pilot NLP tools that auto-populate forms, surface risk indicators, and predict approval likelihood. You will provide the ground-truth data that trains these models to be both accurate and compassionate.
- • Monitor payer feedback loops and denial patterns, then translate insights into proactive documentation improvements. When a new regulation drops, you will be the first to dissect it, update our playbooks, and brief the entire organization within 24 hours.
- • Champion a culture of continuous learning by hosting monthly “Documentation Deep Dives” where teammates dissect challenging cases and share best practices. Your leadership will elevate the entire team’s clinical literacy and storytelling prowess.
- • Contribute to Abby Care’s public voice by anonymizing and repurposing documentation insights into blog posts, white papers, and policy briefs that shape the national conversation on family caregiving.
- • Experience the profound impact of your work through quarterly family spotlights—hearing firsthand how your documentation secured a child’s home nursing hours or an aging parent’s assisted-living placement. These stories will remind you daily why precision matters.
🎯 Requirements
- • 2+ years of hands-on experience in clinical documentation, medical records, or health information management within a healthcare or insurance setting
- • Demonstrated mastery of HIPAA, CMS, and state Medicaid waiver documentation standards
- • Proficiency with EHR systems (Epic, Cerner, or similar), Microsoft Office, and Google Workspace; comfort learning new health-tech platforms quickly
- • Exceptional written communication skills with the ability to distill complex medical information into clear, compliant narratives
- • Nice-to-have: RHIT, RHIA, or CPC certification; experience with long-term-care or home-health documentation; familiarity with ICD-10 and CPT coding
